Enrollment Kit Use the application included in this kit to open an account in CollegeAdvantage. You can download components separately: Program Information, Investment Option Information booklet, New Account Application, at-a-glance Investment Option Guide and Offering Statement.

Account Owner Change Form Use this form to change the Account Owner of your CollegeAdvantage account.

Account Owner Identity Verification Form  If CollegeAdvantage does not already have this information on file, this form must be completed and returned to CollegeAdvantage before any withdrawals can be made from an account.

Address Change Form Use this form to notify CollegeAdvantage of your new address.

Application Use this form to establish a new CollegeAdvantage account.

Contribution Slips Contribution slips contain personalized information and must be mailed to your home.  Click here to order.

Electronic Funds Transfer (EFT) Form Use this form to establish, change or stop automatic cash transfers from a bank account. You can also schedule a one-time debit.

Investment Option Selection Guide Brief summary of CollegeAdvantage investment options and current performance.

Investment Option Addition Form Use this form to add additional investment options to your existing CollegeAdvantage account.

New Beneficiary/Transfer Form Use this form to transfer all or part of your CollegeAdvantage account to a new beneficiary.

Payroll Deduction Form If your employer participates in payroll deduction for CollegeAdvantage, use this form to start, change or stop payroll deduction. You will need your employer's CollegeAdvantage payroll deduction number; see your employer for this number.

Pre-1996 Ownership Amendment Form Use this form if you opened your account before October 1, 1996, and now wish to change control from the Beneficiary to the Account Owner.

Reallocation Form Use this form to reallocate current account assets from existing CollegeAdvantage Investment Options to any available investment options in the program for the same account/Beneficiary.

Successor Addition/Change Form Use this form to add a Successor Owner or change the Successor Owner of  your CollegeAdvantage account.

Trust/Entity Application Use this form to establish a CollegeAdvantage account if you are a trust or other type of entity.

Withdrawal Request Form Use this form to request a withdrawal from your CollegeAdvantage account.
