NEW ACCOUNT APPLICATION 

Opening this account should take about 10 minutes, depending on your choices.

Each child has a separate account. Each account requires a minimum $25 initial contribution to establish the account (CDs require a $500 minimum contribution); we will debit your checking or savings account for the amount you indicate during the online enrollment process. Before beginning, please read the Enrollment Kit, including the Offering Statement and Participation Agreement.You may submit more than one Beneficiary application during this session by following the link at the end of the first successful application submission.

You cannot enroll online if you:       

• are using rollover funds from another eligible account to initially fund your account
• are using UGMA/UTMA funds to make your initial contribution for your account
• are setting up an account with a trust or other entity as the Account Owner.

Information you will need
Before you get started, in addition to personal information, you'll need:

• Social Security numbers for yourself, the beneficiary and successor owner (if named).
• Your driver’s license or state-issued I.D. card.
• The birth date of the beneficiary.
• A check (which displays your bank account number and routing number) so you can fund the account via electronic funds transfer (EFT). You must make your initial contribution via an EFT transaction from your checking or savings account.
• A valid email address.

For security purposes, after you complete the first screen, we will send you a confirmation email; you then use the link provided in the confirmation email to complete the enrollment process. If you do not complete the enrollment process the same day, you will be asked to re-enter your Social Security number and driver's license number when you return to complete the application.

Technical requirements: The online enrollment process works best on Microsoft Internet Explorer version 5.0 or later. The enrollment pages may not display properly on other operating systems.
