The forms and materials available on this site are for the direct account owner only. Customers who opened a CollegeAdvantage account through a financial advisor and have a BlackRock CollegeAdvantage account should contact him or her for the appropriate forms or materials.

ACCOUNT APPLICATION

Use this form to establish a new CollegeAdvantage account. Each beneficiary has a separate account, so an application is needed for each child you wish to enroll.

ACCOUNT INFORMATION CHANGE FORM

Use this form to change your address, your name (due to marriage, divorce, etc.), update Beneficiary information (provide correct or missing SSN, name or date of birth),or to provide Successor Owner information (add, change, update/correct).

ACCOUNT OWNER CHANGE FORM

Use this form to change the Account Owner of your CollegeAdvantage account. This form requires a Notary signature.

ELECTRONIC FUNDS TRANSFER (EFT) FORM

Use this form to establish, change, or stop automatic cash transfers from a bank account. You can also schedule a one-time debit.

INVESTMENT OPTION SELECTION GUIDE

Brief summary of CollegeAdvantage investment options and current performance.

PAYROLL DEDUCTION AUTHORIZATION FORM

If your employer participates in payroll deduction for CollegeAdvantage, use this form to start, change, or stop payroll deduction. You will need your employer's CollegeAdvantage payroll deduction number; see your employer for this number.

PRE-1996 OWNERSHIP AMENDMENT FORM

Use this form if you opened your account before October 1, 1996, are invested in the suspended Guaranteed Savings Fund, and now wish to change control from the Beneficiary to the Account Owner.

REALLOCATION FORM

Use this form to reallocate current account assets from existing CollegeAdvantage investment options to any available investment options in the program for the same account/Beneficiary.

TRANSFER FORM (100% OF ASSETS ONLY)

Use this form if you wish to transfer ALL assets from one CollegeAdvantage account to another. The account receiving the funds must already be established, and you cannot change investment option selection. This form requires a Notary signature.

TRANSFER FORM (REGULAR VERSION)

Use this form to transfer assets from one CollegeAdvantage account to another if any of the following apply: account receiving assets is not yet established; you are transferring a partial amount; you wish to change investment options for the new Beneficiary; you are transferring assets from the suspended Guaranteed Savings Fund. This form requires a Notary signature.

TRUST/ENTITY APPLICATION

Use this form to establish a CollegeAdvantage account if you are a trust or other type of entity.
